Tc-99m PYP Quality Checker — Technetium-99m Pyrophosphate (PYP) for Bones and Heart

— WHY IS THIS NEEDED?
Tc-99m Pyrophosphate (PYP) is a dual-purpose agent:
1. Cardiology (ATTR Amyloidosis): Specifically binds to transthyretin deposits in the myocardium. "Gold standard" for non-invasive diagnosis of ATTR cardiomyopathy.
2. Orthopedics/Oncology: Chemisorbs on bone hydroxyapatite. Used for detecting metastases, fractures, and osteomyelitis.
• Historically used for detecting myocardial infarction (necrosis), but this niche is now occupied by other methods.

⚠️  CRITICAL:
• Radiochemical purity ≥95% — Low RCP reduces contrast. For cardiology, absence of colloid is particularly important because the liver is located directly beneath the heart. Liver background can mimic or mask uptake in the heart.
• Aluminum Breakthrough ≤10 ppm — CRITICAL PARAMETER. Aluminum ions react with pyrophosphate, forming insoluble particles. This leads to uptake by RES cells of liver and spleen ("hot liver"), which completely distorts the image of the heart and lower ribs.
• Free pertechnetate ≤5% — Artifacts in thyroid and stomach area.
• Reduced/Hydrolyzed Tc ≤5% — Colloid accumulating in liver.
• pH 4.0–7.5, Endotoxins ≤175 EU/vial, Sterility — standard requirements.

💡 Usage tips:
1. Aluminum test is mandatory before preparing the batch, especially for cardiac studies.
2. For ATTR amyloidosis diagnosis, high signal-to-noise ratio between heart and bloodstream/bones is required. Impurities degrade this ratio.
3. PYP is stable for 6-8 hours after preparation.
4. When assessing the heart, consider accumulation time (images usually taken 1 hour post-injection).

⚠️ Note: Unlike MDP, PYP has higher affinity for calcifications in soft tissues and amyloid deposits. However, its chemical sensitivity to aluminum is as high as other phosphate agents. "Hot liver" due to aluminum is a frequent cause of failed PYP studies.
