— WHY IS THIS NEEDED?
Thallium-201 (T½=73 hours) is a unique radiopharmaceutical that behaves as a potassium (K+) analog:
• Actively transported into cells via Na+/K+-ATPase
• Cardiology: Gold standard for assessing myocardial viability (hibernating myocardium). Uptake depends on blood flow and metabolic activity of cells.
• Oncology: Used for imaging parathyroid tumors (adenomas), some brain and lung tumors, as they actively accumulate thallium.
• Emits mercury X-rays (69-83 keV), suitable for gamma cameras.
⚠️ CRITICAL:
• Radiochemical purity ≥98% (Tl+ Ion) — DRUG MUST BE IN THE FORM OF THALLOUS CHLORIDE (TlCl). Any complexes or colloidal forms will not be taken up by cells via potassium channels. This is a fundamental difference from technetium drugs.
• Radionuclidic purity ≥99.5% — Presence of other thallium isotopes is unacceptable.
• Thallium-200 (T½=26.1 h) impurity ≤0.5% — Tl-200 emits high-energy gamma quanta, increasing patient radiation dose and creating image noise.
• Thallium-202 (T½=12.3 d) impurity ≤0.5% — Long-lived impurity creating long-term radiation burden.
• pH 4.0–7.5, Endotoxins ≤175 EU/vial, Sterility — standard requirements.
• Heavy metals ≤10 ppm — Standard purity control.

💡 Usage tips:
1. Thallium-201 is produced in a cyclotron (protons on lead or bismuth target, or enriched thallium). Impurity control depends on target purity and beam energy.
2. The drug is stable for a long time (weeks), but usually used within a few days after release due to decay.
3. For cardiology, stress and rest tests are performed with an interval, using thallium redistribution.
4. Tl-201 has a higher radiation dose to the patient compared to Tc-99m agents, so it is used strictly according to indications (viability assessment).
⚠️ Note: Thallium-201 is not just a blood flow indicator, but an indicator of METABOLIC ACTIVITY and cell membrane integrity. If a cell is dead (necrosis), it will not accumulate thallium, even if blood flow is restored. This makes it indispensable for assessing myocardial viability before revascularization.input.csv
