Rubidium-82 Chloride Quality Checker — Rubidium-82 Chloride for Myocardial PET Perfusion

— WHY IS THIS NEEDED?
Rubidium-82 (T½=75 seconds) is an ideal radiopharmaceutical for myocardial perfusion assessment using PET:
• Allows rapid studies without an on-site cyclotron (generator system)
• High image quality due to positron decay
• Short half-life reduces patient dose after procedure completion
• Used for diagnosis of coronary artery disease, myocardial viability assessment

⚠️  CRITICAL:
• Strontium-82 (Sr-82) breakthrough ≤20 Bq per 1 MBq Rb-82 — Sr-82 is the parent isotope with a 25-day half-life. If it enters the patient's body, it accumulates in bone tissue (like calcium) and creates significant radiation burden to red bone marrow for a long time. This is the MAIN safety parameter of the generator.
• Strontium-85 (Sr-85) breakthrough ≤500 Bq per 1 MBq Rb-82 — Sr-85 (T½=65 days) is also a production impurity of Sr-82. Although less dangerous than Sr-82, its accumulation is also undesirable.
• Radiochemical purity ≥95% — Rubidium must be in chloride form (Rb+ ion), which is a potassium analog and is taken up by myocytes via Na+/K+-ATPase.
• pH 4.0–8.0 — for safe intravenous administration.
• Appearance: Clear, colorless solution, free of particles.
• Endotoxins ≤175 EU/dose — standard for injectables.
• Sterility — MANDATORY.

💡 Usage tips:
1. Strontium breakthrough test is performed regularly (daily or before first elution of the day) using a dose calibrator with ionization chamber and lead shield (Sr-82 emits gamma radiation, Rb-82 mainly positrons/annihilation gammas, but method requires specific equipment or chromatography).
2. If Sr-82 breakthrough exceeds limit, generator must be immediately taken out of service and disposed of as radioactive waste.
3. Due to short half-life of Rb-82, all tests except strontium breakthrough and appearance are often retrospective or controlled by manufacturing process (sterility, endotoxins).
4. Eluate is administered immediately after elution, as activity decays rapidly.

⚠️ Note: Unlike other radiopharmaceuticals, Rb-82 quality directly depends on generator column integrity. Strontium breakthrough is not just an impurity, but a sign of separation system failure, making use of such generator dangerous for patient life due to leukemia risk (bone marrow irradiation).
