Gallium-68 FAPI Quality Checker — ⁶⁸Ga-FAPI for PET imaging of tumor stroma

ℹ️ Utility checks quality of FAPI (Fibroblast Activation Protein Inhibitor) radiopharmaceutical:
    • Specific activity (≥50 GBq/μmol)
    • Radiochemical purity (≥95%)
    • Germanium-68 impurity (≤0.001%)
    • Free gallium-68 (≤5%)
    • pH, endotoxins, sterility, heavy metals, aluminium

⚠️ IMPORTANT: ⁶⁸Ga-FAPI is in clinical trials phase (2019-2024).
Not approved by FDA/EMA as standard of care. Intended for:
    • Research use
    • Compassionate therapy
    • Clinical trials

Usage:
Gallium68FAPIQualityChecker.exe → demo mode (console output)
  Gallium68FAPIQualityChecker.exe input.csv output.json → evaluate your data

Input format:

— WHY IS THIS NEEDED?
FAPI (Fibroblast Activation Protein Inhibitor) — promising ligand for imaging and therapy of solid tumors:
• Binds to FAP protein expressed by cancer-associated fibroblasts (CAFs) in tumor stroma
• FAP expression found in 90%+ solid tumors (pancreas, stomach, liver, colorectal cancer)
• Practically absent in healthy adult tissues (only in healing wounds, placenta)
• ⁶⁸Ga-FAPI PET shows superiority over FDG for:
  — Pancreatic ductal adenocarcinoma (2-3x higher SUVmax)
  — Primary liver tumors (better HCC visualization)
  — Gastric cancer (superior primary tumor detection)

⚠️ CRITICAL:
• Specific activity ≥50 GBq/μmol — critical for optimal tumor stroma uptake
• Germanium-68 impurity ≤0.001% — CRITICAL! Germanium-68 has 271-DAY half-life!
Even 0.0011% creates chronic radiation burden without diagnostic benefit.
• Free gallium-68 ≤5% — UNBOUND gallium accumulates in bones, masking metastases
• Aluminium <10 ppm — concentrations ≥10 ppm cause bone uptake artifacts on PET
• pH 4.5–7.5 — critical for blood compatibility during IV administration
• Endotoxins ≤175 EU/vial — critical for IV administration!
• Sterility — MANDATORY for all parenteral forms!

Key features:
• Support for different tumor types with clinical evidence display
• Critical warnings: "CHRONIC RADIATION BURDEN!", "BONE MARROW UPTAKE!"
• Automatic verification against 2019-2024 research specifications
• Demo data for pancreatic cancer (most studied type)

Critical parameters:
• Specific activity: ≥50 GBq/μmol (same as ⁶⁸Ga-DOTATATE)
• Radiochemical purity: ≥95.0%
• Germanium-68 impurity: ≤0.001% (CRITICAL: 271-DAY HALF-LIFE!)
• Free gallium-68: ≤5.0% (bone metastasis masking)
• pH: 4.5–7.5 (blood compatibility)
• Endotoxins: ≤175 EU/vial (LAL test)
• Sterility: no microbial growth
• Heavy metals: ≤10 ppm
• Aluminium: <10 ppm (IMAGE ARTIFACTS!)
• Residual ethanol: ≤5000 ppm

⚠️ Product status (2024):
• Not approved by FDA/EMA as standard of care
• Used in Germany, China, Russia clinics for compassionate therapy
• 50+ clinical studies, 2000+ patients
• Approved for clinical use in China since 2022
• Key studies: Kratochwil et al. (J Nucl Med 2019), Giesel et al. (EJNMMI 2022)

💡 Usage tips:
1. For research use ensure protocol approved by ethics committee
2. Germanium-68 impurity >0.001% makes batch unsuitable even for research
3. Free gallium-68 >5% requires re-purification
4. Document all measurements for clinical trial reporting

⚠️ Limitations:
• Parameters based on current research and may be refined upon regulatory approval
• Utility does NOT replace official pharmacopoeial quality control
• Intended for research use and compassionate therapy
• Not a medical device

7. FS — functional specification

IDFunctionImplementation description
FS-001CLI entry pointThe executable Gallium68FAPIQualityChecker.exe supports demo mode and input.csv output.json processing mode.
FS-002CSV parserThe import module reads CSV, validates header, column presence/order, value count and encoding. Decimal values are expected with a dot separator.
FS-003Field conversionEach column is converted to the expected type: identifier, text, decimal number, date/time or boolean flag.
FS-004Domain rule engineFor Gallium68 FAPI, explicit rules are applied: range, minimum, maximum, absence of prohibited flag, data completeness or calculation-based check.
FS-005Criticality handlingCritical violations produce FAIL; non-critical deviations and incomplete data produce WARNING; full conformance produces PASS.
FS-006JSON writeroutput.json stores overall status, per-parameter results, source values, warnings, failures and diagnostic messages.
FS-007Integration contractThe CSV → JSON format is stable for invocation from LIMS/ELN/MES, scheduled task or wrapper service.
FS-008Error handlingSchema error, missing file, non-numeric value or JSON write failure returns diagnosable error without silent PASS.

8. output.json contract

The output file must be suitable for automated processing, audit review and correlation with the source input.csv row.

10. QA/QC, CSV and change control

  • Before production use, the client records executable version, checksum, specification/monograph version, test CSV, expected JSON and IQ/OQ results.
  • Column names must not be changed without updating the validator and test scenarios.
  • The source CSV, output.json and utility version should be stored together as an evidence package.
  • Any change in control rules must go through change control and repeated OQ scenario verification.
